The Debt & Mental Health Evidence Form (DMHEF), along with the appropriate Consent Form, is a tool that will enable debt advisers and creditors to request clear, relevant and comprehensive information in a standard format from health and social care professionals, as appropriate to each given situation. Creditors on receipt of the completed Form should be in a better position to make appropriate decisions about the debts of their customers. Please remember the Consent Form is to enable the individual experiencing mental health problems that affect their ability to manage their money, to give their consent for their health professional to complete the DMHEF on their behalf which will then be shared with their creditors.
This third version of the Debt & Mental Health Evidence Form was launched on 28th November 2012. Please remember it should only be used when the individual is experiencing mental health problems that seriously affect their ability to manage their money. It is very important to remember to read the appropriate Guide for Users before you use the DMHEF for the first time.
The DMHEF and The Consent Form have been approved by the Information Commissioner’s Office and carry the Plain English Campaign’s Crystal Mark.
